Official Rules – “5-Star Smile” Oral-B Giveaway

1. Sponsor

This sweepstakes (“Sweepstakes”) is sponsored by Oakland Dental Associates, located at 2648 International Blvd, Ste 602, Oakland, CA 94612.

2. Eligibility

The Sweepstakes is open only to legal residents of California who are existing patients of Oakland Dental Associates, and who have had an appointment between Jan 1, 2024 and Dec 31, 2025. Entrants must be 18 years of age or older at the time of entry. Employees of the Sponsor and their immediate family members are not eligible to participate.

3. Sweepstakes Period

The Sweepstakes begins on November 15, 2025 at 12:00 PM PST and ends on December 31, 2025 at 11:59 PM PST. All entries must be received by the end of the Sweepstakes Period.

4. How to Enter

Eligible participants may enter the Sweepstakes through one (1) of the following methods:

a. Review Entry

1. Leave a 5-star review for Oakland Dental Associates on Google or Yelp.

2. After posting your review, show it to the front desk receptionist at your next visit.

3. The receptionist will record your entry in the sweepstakes on your behalf.

b. Alternate Method of Entry (No Review Required)

To enter without leaving a review, send an email with your full name, phone number, and subject line “Sweepstakes Entry” to: oaklanddentalassociates@gmail.com The email must be received before the end of the Sweepstakes Period.

Only one entry per person is allowed, regardless of entry method.

5. Prize

One (1) winner will receive an Oral-B iO Electric Toothbrush (approximate retail value: $199). The prize is non-transferable and cannot be exchanged for cash or any other alternative.

6. Winner Selection and Notification

The winner will be selected in a random drawing from all eligible entries on or about January 7, 2026. The odds of winning depend on the number of eligible entries received.

The winner will be notified via email or phone within five (5) days of the drawing. If the winner cannot be contacted within seven (7) days, or is disqualified, the Sponsor reserves the right to select an alternate winner.

7. General Conditions

By entering, participants agree to be bound by these Official Rules and the decisions of the Sponsor, which are final. Sponsor reserves the right to disqualify any entrant who tampers with the entry process or violates these rules.

This Sweepstakes is subject to all applicable federal, state, and local laws and regulations. Void where prohibited.

8. Privacy

Information collected from entrants is used only for the purpose of administering the Sweepstakes and will not be sold or shared with third parties.

9. Limitation of Liability

By entering, participants agree to release and hold harmless [Your Practice Name] from any and all liability arising from participation or the acceptance, use, or misuse of any prize.

10. Winner List

To request the name of the winner, send an email to oaklanddentalassociates@gmail.com with subject line: “Sweepstakes Winner Request” by February 6, 2026.
